and "Membership Application" can be filled out online,
then click the print button at the top of the form's page
to print it and mail it in to the IHBA office. The "Medical
Certification Form" is only meant to be printed, then
filled out offline, click the print button at the top of the
page to print it and take it to your doctor to fill out.
